Lifepath Colleague Wellness Program

Lifepath is Northern Arizona Healthcare’s free colleague wellness program. The lifepath team is dedicated to helping colleagues and their dependents achieve their highest level of health. Each year, the team designs the program and offers services in response to current colleagues’ needs, feedback and the latest research and best practices in workplace wellness. The program includes an action plan to earn significant rewards for participation.

Community Care Management

Community Care Management provides comprehensive services to help colleagues and dependents living with chronic conditions. Free services include comprehensive assessment to identify individual needs; development of a collaborative care plan with measurements, goals, and sustainable results; intensive one-on-one support; scheduled home visits; phone support and self-management classes.

Health Coaching

Health coaching is a free, confidential partnership between you and a health coach to help you achieve your personal wellness goals and lifestyle changes. Health coaching taps your inner wisdom to identify your values and transforms your goals into action. This process incorporates physical, mental, spiritual and an emotional approach to improve your life balance and optimize health. Health coaching is designed to support you and hold you accountable for your goals. In addition, our experienced coaches provide a number of programs to help you reach and maintain optimal health in your work and home environment. You can accumulate points for participating, resulting in significant financial reward through the lifepath colleague wellness program. Win-win!

Employee Assistance Program

NAH contracts with Optum Employee Assistance Program, which offers financial and legal services; child/elder care placement assistance; access to message boards for a safe and friendly environment to support one another and share information; and self-assessments, tools and calculators on a variety of topics ranging from work-life balance to weight loss. There is unlimited access to a telephonic counselor, available 24 hours a day, year-round. The program also offers six free in-person counseling sessions per incident.

Flagstaff Medical Center’s Behavioral Health Services Outpatient Office

FMC BHS employs experienced counselors with graduate degrees in the fields of counseling, psychology and social work. Counselors can assist with family or relationship issues, drug or alcohol use, grief, loss or trauma, feeling depressed or anxious, anger management, interpersonal issues, communication issues, or stress at work or home. Office visits are available in a confidential setting with special attention to ensure a good match between client and counselor, and to ensure privacy. Six, free, in-person counseling sessions are available for each individual issue.

EntireCare Employee Fitness
