Palliative care at NAH

A partnership of patient, family and specialists

Palliative care is a team approach to care. The core team includes doctors or nurse practitioners, nurses, and social workers. Pharmacists, chaplains, interpreters and others may also be a part of the team. Our team spends as much time as necessary with you and your family. We partner with you, your family and your other doctors. We support you and your family every step of the way, not only by controlling your symptoms, but also by helping you understand your treatment options and goals.

Working together with your primary doctor, our palliative care team provides:

  • Close communication.
  • Expert management of pain and other symptoms.
  • Help navigating the healthcare system.
  • Guidance with difficult and complex treatment choices.
  • Matching treatment to the goals of the patients and families.
  • Empowering patients and families to determine their goals.
  • Helping patients and families to have a voice in healthcare decisions.

Consultation service

The palliative care consult team is available to all patients admitted to Flagstaff Medical Center. This interdisciplinary team specializing in palliative care works with patients and their primary doctors to help develop a continuity plan for palliative care.

Reasons to request a palliative care consult include:

  • Pain and symptom management.
  • Counseling/guidance about care options in the face of serious illness.
  • Discussing goals of care.
  • Assistance in developing a palliative care discharge plan.

Supportive Care Clinic

The Supportive Care Clinic helps patients to manage serious illness and its symptoms. 

Services include:

  • Pain and symptom management.
  • Counseling/guidance about care options in the face of serious illness.
  • Discussing goals of care.
