How do I enroll in the patient portal?

There are two easy ways to enroll in the patient portal.

1. Self-enrollment:

Self-enrollment is available to patients who are 18 years or older and who currently do not have access to MyNAHealthcare.

  • Click the button below to begin.
  • Complete the online form.
  • The CMRN number is your eight-digit NAH account number, which can be found on your billing statement; can be given to you by a clinic or registration staff member; or by calling the NAH IT Service Center. The CMRN number is your eight-digit NAH account number, which can be found on your billing statement; can be given to you by a clinic or registration staff member; or by calling the NAH IT Service Center and requesting an invitation.

2. Request an email invitation:

Ask a registration or clinic staff member to send you an email invitation to join the portal, then follow the instructions in the email. Sometimes portal invite emails can get caught in your SPAM or junk mail, check these folders if you don’t receive an email invitation after you have requested it.
