As a reminder, all insurance plans are different; we recommend calling your insurance plan to confirm the procedures/services you are scheduling are covered under your benefits plan. Some services may be covered in-network, while others may not. Please check with your insurance plan to determine your specific coverage.
AHCCCS (Medicaid) insured patients
Starting in April 2023, the state-funded Arizona Health Care Cost Containment System (AHCCCS) began to require regular renewals for participants. You must take action to renew your coverage.
