The goal of the Patient Placement Center is to streamline admissions to Flagstaff Medical Center and facilitate communications between physicians to better meet the needs of referring physicians and patients.

  • We are available 24 hours a day, seven days a week
  • We are staffed by registered nurses
  • We handle all admissions and transfers to FMC
  • We offer order writing for easier processing
  • We prefer to speak with the physician ordering tests

Please have the following information ready when you call:

  • Patient’s name, age and date of birth
  • Diagnosis
  • Admission location
  • Referring physician’s name and contact phone number
  • Fax a face sheet for the patient

Inpatient and observation admissions

  • Initial patient information and appropriate bed assignment
  • Transcribe orders for admission
  • Assistance in arranging for testing prior to bed placement
  • Hospitalists Services will be contacted through One-Call Admissions

Referrals from outlying facilities

  • Facilitate physician-to-physician consultations with specialists for physicians from other institutions
  • Arrange for transfer of patient for admission or diagnostic procedures
  • Facilitate air and ground transport
  • Facilitate communication for outlying physician with other departments within FMC
  • Facilitate communication between outlying institutions and FMC nursing staff
  • Provide follow-up communication with outlying physicians about their patients

In-house transfers

  • Initial notification of transfer orders from unit to unit

Outpatient diagnostic procedures

  • Facilitate communication with Imaging, Endoscopy and other outpatient departments for outlying patients
  • Facilitate appropriate patient observation, medical and nursing, while patient is at FMC

Trauma Services

All trauma patients can be referred to FMC through the Patient Placement Center/Physician-to-Physician Referral Line, and are accepted by the emergency physician on duty, 24 hours a day. Trauma specialists will be consulted by the emergency physician according to patient need. There is no requirement to contact the specialty surgeon directly, unless the referring physician desires.
