There are several options for enrolling in MyNAHealthcare. To ensure a successful experience, please keep the following in mind when you’re ready to sign up.
- Northern Arizona Healthcare patients who are 18 or older can self-enroll to create their own MyNAHealthcare account.
- Patients aged 13 to 17 may request an email invitation to create their own MyNAHealthcare account, but they cannot self-enroll.
- Patients under 13 cannot have their own MyNAHealthcare account; however, a parent or permanent legal guardian can request proxy access to view the child’s medical information using MyNAHealthcare.
- Patients can opt to give a family member or caregiver access to their NAH medical information through the patient portal. This is known as naming a “Proxy.” A Proxy can view medications, procedures, lab results, and more. A Proxy can also manage appointments and send secure messages to the patient’s care team.
- If proxy access is given to a parent or legal guardian for a child under 13, that access will automatically end on the child’s 13th birthday. At age 13, the child is eligible to create their own MyNAHealthcare account and decide who can view their medical information by approving or denying proxy access.
Self-enrollment
If you are currently a NAH patient and do not have access to MyNAHealthcare, are 18 and older, and would like to self-enroll:
- Complete the self-enrollment form by adding your name and date of birth.
- Choose one of three ways to verify your account:
- Email address
- CMRN or NAH account number (see below)
- Text message
Please note: The email address or mobile phone number you enter must match the one you provided when you registered for your most recent appointment. Your CMR or Community Medical Record Number is your NAH account number, which can be obtained by contacting your provider’s office or the Patient Portal Helpline.
Request an email invitation
Patients aged 13 and older can sign up for MyNAHealthcare by calling the Patient Portal Helpline at 877-624-7678 to request an email invitation. Follow the instructions in the email to create your account. Sometimes, portal invitation emails can be caught in your spam or junk mail. Check these folders if you don’t receive an email invitation after requesting it.
Authorize a Proxy
If you would like to give someone proxy access to your NAH medical information, OR if you are a parent, family member or caregiver who would like to request proxy access:
- Please complete the Patient Portal Proxy Request and Authorization form and submit the required documents.
- Both the patient and the Proxy must sign the authorization form.
- The request form, list of required documents, and instructions on how to submit them are available at NAHealth.com/proxy.
