Northern Arizona Healthcare is conducting its three-year Community Health Needs Assessment and your feedback is very important.
NAH’s Mission is “Improving health, healing people” and your input gives us direct insight on where we stand, as a community, on various health issues such as access to medical care; having a primary care doctor; and health behaviors.
The survey takes about 15 minutes to complete and is completely confidential.
