Your visit: What you need to know

Before your appointment

It’s important for your healthcare provider to have up-to-date information. Please call the clinic and ask if any paperwork is needed before your appointment. 


What to bring with you

  • Insurance card or information
  • Photo identification
  • Updated medication list, including over-the-counter medications, vitamins and supplements.

Please make sure you have obtained the necessary authorization from your primary care provider if your insurance carrier requires it. 


If you need hospitalization

Needing hospital care can feel overwhelming. By identifying your needs early on, we can map your progress and plan your discharge to keep you moving toward recovery.

Your comprehensive healthcare plan will address any additional needs, such as homecare services, behavioral healthcare or placement in an assisted living facility.


Insurance and billing information

Payment is expected at time of service. Northern Arizona Healthcare’s clinics accept Medicare and most other insurance plans. In addition, patients may access financial assistance through the Arizona Cost Containment System, or AHCCCS. For more information, visit AHCCCS online.
