Your visit: What you need to know

We recommend you complete the new patient paperwork and bring it with you to your appointment. Please call the clinic to obtain a packet. Be sure to list all your current medications and supplements, including dosages. It’s important for your healthcare provider to have up-to-date information.


What else to bring with you

  • Photo identification

Please make sure you have obtained the necessary authorization from your primary care provider if required by your insurance carrier. 


Insurance and billing information
