Your visit: What you need to know

We understand your time is valuable. We make every effort to remain on schedule. We look to our patients to also be prompt and assist us by staying on time. If you are more than 20 minutes late, your appointment may be cancelled in fairness to the patients who have arrived for their appointments on time. It is also important to be very clear what your needs are when you are scheduling your appointment so we may allow enough time for your appointment.

Due to the nature of obstetric practices, physicians may be called out suddenly for deliveries and emergencies. We will make every effort to notify you as soon as possible if your appointment will be delayed or cancelled. We appreciate your patience and understanding when these situations arise.

When making appointments, please leave us a phone number where you think you will reachable just before the visit.  We also recommend you call the office for a status check before leaving.

Please notify us if you are not able to keep your appointment. A 24-hour notice is very much appreciated. This allows us to contact someone who may need that appointment time.


Before your appointment

We recommend you complete the personal medical history form before your visit. Be sure to list all your current medications and supplements, including the dosage. It’s important for your healthcare provider to have up-to-date information.  

Please call the clinic for your paperwork in advance of your appointment.


What to bring with you

  • Driver’s license or other photo I.D.
  • Insurance card(s)
  • Completed personal medical history form
  • Method of payment
  • Medical records pertinent to your care (ultrasound reports, lab results, records from another physician)

Insurance and billing information

Payment is expected at time of service. Northern Arizona Healthcare’s clinics accept Medicare and most other insurance plans. In addition, patients may access financial assistance through the Arizona Cost Containment System, or AHCCCS. For more information, visit AHCCCS online.
