What you need to know before your primary care visit
Before your next primary care visit, we will send you a reminder by text or email. The message will ask you to confirm your appointment. It will include clear instructions for how to register in advance.
Filling out your information before your visit makes your appointment faster and easier. That way, you can get back to what matters most.

By registering before your visit, you can:

- Help ensure your information is complete and accurate.

What to bring with you
Even if you pre-register, please bring your:
- Photo ID, such as a driver’s license or state-issued ID card.
- Insurance card or insurance information.
- List of your current medications.
- Notepad and pen, if you’d like to take notes during your visit.
