Sports physicals in Camp Verde for $10

Stop by the Northern Arizona Healthcare Orthopedic & Spine Institute at 1298 W. Finnie Flat Rd in Camp Verde on July 22 from 3:00 pm-6:00 pm. We’re hosting our $10 sports physicals for school-aged kids to meet the pre-season physicals requirement for participating in school-sponsored sports.
Before arriving, please fill out forms 15.7-A and 15.7-B. The cost is $10.00 cash (no credit cards accepted).
Proceeds from the event will be donated to the school where the student is enrolled and will benefit the Sports Medicine Program.
