NAH’s healthcare preference survey

Patient- and Customer-Focused Excellence is one of the guiding principles in our plans for the future. In order to improve the health of our communities, and to gain your trust, Northern Arizona Healthcare must provide you with care and an experience that are amazing. The first step in achieving this is to involve you in the conversation about health and wellness, as well as access to it, in our communities. By continuing to build our relationship with you, we hope to strengthen our trust and partnership in health and wellness. 

One way we’re reaching out to our communities is by conducting a survey to better understand how you prefer to receive information about wellness and our services; your perceptions of our services; how you prefer to access services and your personal health information; what you believe we do well and what needs to improve; and where you receive services outside of our communities.

A paper survey was mailed to a few households randomly selected and an online survey is available to you and the rest of the community. We consider your responses critically important to planning for the future of healthcare in Northern Arizona. 

Northern Arizona University’s W.A. Franke College of Business specializes in survey research, and they are partnering with us on this study. Be assured that your responses are completely confidential and all data will be reported only in the aggregate; you can never be individually identified. Everyone who returns the survey will be entered into a drawing for one of two iPads being offered by our NAU survey research partner.
