Coconino County  

Flagstaff Elks Lodge - for people 18 years and older who live and/or work in Coconino County.

Yavapai County

The Hub, Sedona - ALL Yavapai County residents that are 18 years old and older are now eligible to be vaccinated.

Northern Arizona Healthcare and their vaccination site partners are unable to accept phone calls to schedule. YOU MAY ONLY SCHEDULE ONE APPOINTMENT AT A TIME. If you are making an appointment for more than one person, you will need to complete each appointment separately. LOG OUT after creating each appointment, close the link and start new each time. Failure to follow these directions will create duplicate appointments under the same name.
