Arizona State Board of Nursing licensure information

All nurses must have a current Arizona license or an Arizona temporary permit to practice in the state of Arizona. Applications for licensure must be obtained from:

It is preferable that the nurse complete the application process before arrival in Arizona. An offer of employment will not be made without verification of a valid Arizona temporary nursing permit or permanent license.


Credentials needed to process licensure:

1. Current nursing license from another state. A photocopy is acceptable. If no current license, submission of a verification record of inactive status will be acceptable.

You will still be expected to contact your current state of licensure board and pay a fee for them to send written verification to the Arizona Board.

2. The application fee for Arizona must be in the form of a money order or cashier’s check made payable to the Arizona State Board of Nursing.

3. A black and white photo, 2 x 2, and not over one year old must be submitted with application.


Temporary Permits:
A temporary permit can be issued to nurses who have applied for licensure in Arizona and have met the application criteria. There is a fee for the temporary permit in addition to the fee for applying for permanent licensure. This fee can be paid at the same time the application is made for licensure. The process for approval for a temporary permit takes ten business days.

CANADIAN NURSES

* When submitting your application for Arizona licensure, include signed, sealed, and notarized copies of transcripts from your nursing school. 
* Also include a copy of your Canadian RN registration OR copies of your CNAT scores (must have been taken in English).
* NCLEX exams are required for Arizona licensure.
